Tên chia thành hai dòng với phần họ tô màu cyan, các mục được đánh số 01, 02, 03 theo thứ tự trong CV, một cột hẹp bên phải chứa phần giới thiệu, kỹ năng và học vấn, còn đầu trang có các đường mạch điện và mũi tên nhọn, kèm vài từ giãn chữ rộng ở các góc trang.
